Accidental Medical Expenses Claims
Things you will need:
  1. Immediate Notification/Submission of claim forms
  2. Submission of claim’s supporting documents
    1. Evidence of Medical bills incurred as a result of accidental bodily injury sustained.
    2. Original Police Investigation Report if death occurred as a  result of accident.
    3. Photocopy of the Means of identification of the beneficiary
    4. Birth Certificate of the beneficiary

Death Claims
Things you will need:
  1. Immediate Notification/Submission of claim forms
  2. Submission of claim’s supporting documents
    1. Medical Certificate of death (original to be sighted and returned)
    2. Burial Certificate issued by the National Population commission (original to be sighted and returned)
    3. Original Police Investigation Report if death occurred as a  result of accident.(Optional)
    4. Photocopy of the Means of identification of the beneficiary
    5. Birth Certificate of the beneficiary

Disability Claims (Permanent & Temporary)
Things you will need:
  1. Immediate Notification/Submission of claim forms
  2. Submission of claim’s supporting documents
    1. Hospital Medical Report confirming the degree of disability
    2. Original Police Investigation Report if death occurred as a  result of accident.
    3. Photocopy of the Means of identification of the beneficiary
    4. Birth Certificate of the beneficiary
